A real patch is done from the inside of the tire, not a plug jammed in from the outside. Ours is a combination plug-patch: we dismount the tire, buff the inner liner, vulcanize a proper patch with a stem running through the puncture. That's the DOT-accepted method. External plugs can work short-term but they don't seal the inner liner. Air eventually migrates into the belts and the tire can fail. A proper patch seals from the inside — that's why shops quote it higher and why it's the right fix.
A properly patched tire lasts the remaining life of the tire. We've patched tires that went another 30,000 miles without issue. The patch is not the weak point.
Sidewall punctures, shoulder punctures, anything larger than roughly 1/4", and tires that have already been driven flat long enough to damage the sidewall — those get replaced, not patched. We'll tell you straight.
